With Collaborate’s External Forms feature, sending customized forms, such as Surveys, to Clients has never been easier. 


Start by generating a link to the form via the “Generate” External Forms action.

Once the link has been generated, two options will be available to you. One option is to manually send a link to the external form by clicking “Copy URL” and pasting the link into an email correspondence that you are sending to your Client.


The second option is to send the link directly out of Collaborate and to the Client via SMS message or automated email. This is done by clicking “Send URL”.

Once the Client receives the link, they can click it to fill out the form. 

Once the form has been completed and submitted, the information on the form is automatically saved into Collaborate on that Client's case profile, where it can be Viewed as needed.


In addition, any information from External Forms can be reported on via Collaborate’s robust reporting feature.
